North Lincolnshire’s Plan for Health and Social Care Integration
Our Community First Strategy is North Lincolnshire’s all age plan for health and social care integration and transformation. It was launched at the ‘Making It Real’ health and social care integration event in May 2023.
The Strategy describes our shared ambition for North Lincolnshire to be the best place for all our residents to be safe, well, prosperous and connected; experiencing better health and wellbeing.
The Strategy is centred around the three outcomes of:
- People are safe and have good quality provision
- People have urgent needs met quickly
- People are enabled to keep well.
All health and social care organisations in North Lincolnshire have signed up to the Community First Strategy 2023.
Our Community First Annual Report shines a light on the work that’s taken place during 2023-24 to ensure that health and social care in North Lincolnshire achieves the three outcomes listed above. The Annual Report describes a clear focus on transforming how people are supported through increasingly integrated approaches. It also sets out a plan for further and faster transformational change across the health and social care system. What difference will Community First make?
We will know that Community First is making the difference when everyone who needs health and social care support feels and says things like:

“I am supported to plan ahead for important changes in my life that I expect to happen”.

“I can get information and advice about my health and support”.
“I have care and support that lets me live how I want to live”.
“When I change services, there is a plan for what happens next and who does what”.
“I can get information on how to look after my overall wellbeing”.
“I am supported to understand any risks and keep myself safe”.
