The Family Carers Team provide dedicated support to carers who are believed to have eligible unmet needs.

Describe a typical day in the life of the whole team
- Answering phone calls and responding to queries
- Triage calls, booking assessments and reviews
- Promoting independence and support for past carers
- Completion of emergency plans
- Applications for Direct Payments
Describe a typical day of a job role within your team
- Visiting carers through pre-arranged visits to complete assessments or reviews, spend the time getting to know them, their caring role and what is important to them. Support them in considering what they can do to lighten their load and help them to balance their caring role more. Consider their areas of interest to ensure their wellbeing is promoted on a regular basis and what other community-based services can support the person to meet their needs. Work up a rough plan of how their support as a carer will be managed going forward
- Return to the office and type up the assessment, making referrals to other services as you go through the assessment. Link in with other colleagues as needed and support those on duty to respond to any contacts coming in from carers
