Information about Home First Residential in North Lincolnshire – providing short-term care and reablement.

About Home First Residential

The Home First Residential (Intermediate Care Centre) is part of North Lincolnshire Council’s Adult Social Care support offer. It is located within Sir John Mason House in Winterton. The centre provides time-limited, rehabilitation and reablement support to adults (18 years and over) who reside, or are registered with a GP, in North Lincolnshire.

We aim to provide support to improve mobility and daily living skills which in turn can:

  • help prevent unnecessary admission to hospital
  • help with timely discharge from hospital
  • prevent avoidable admission to long-term residential care – helping you to live in your own home for longer

You may need support after a stay in hospital, or a period of illness, to regain the physical strength and daily living skills needed to restore your independence.

The service can also be accessed if you are unwell and live in the community but would benefit from rehabilitative support in the Home First Residential, to help you to continue to live confidently at home.

Support is tailored specifically to your needs. We provide a uniquely personalised level of therapy and care. This helps speed recovery, improve confidence and get you living as independently as possible in your own home as quickly as possible.

The team includes staff from social care and health services, including occupational therapists, physiotherapists, district nurses and general practitioners. They provide a mixture of social care support and health therapies to help you achieve your goals to live as independently as possible. This may include:

  • support to improve mobility and health needs
  • help with daily living activities and practical tasks
  • building confidence to carry out these activities independently
  • support to get back home after a stay at Home First Residential

Day one

We will encourage you to get out of bed, get dressed and get moving. People who do this from an early stage improve faster and are able to get home quicker.

A personalised plan of therapies and assistance to regain physical strength, daily living skills and the confidence you need to live in independently will be designed at a welcome meeting. We will also discuss a plan for getting home, and what needs to be done for that to happen. We will review and discuss how you will manage at home, and review plans with you and your family or carers so that support is always tailored to your care needs and wishes.

Your care

When you first arrive at the Home First Residential, our staff will work with you to draw up a support plan. Your support plan will identify what support you require to improve your physical independence and daily living skills. It will also set out clearly the goals you would like to achieve towards improving wellbeing and independence with an ultimate goal of returning home.

Your plan will be continuously reviewed as your needs change to ensure you are receiving a level of care that is right for you and you are being supported to carry out tasks for yourself.  We will work with you to ensure that reasonable adjustments are made to your home to allow you to return home as quickly as possible.

You will always be fully involved in developing your support plan and we will actively seek and listen to any opinions, choices and input you have. Your circle of support is also welcome to be included and share their views. It is important to us that you have choice and control over the service you receive, so we can keep your individual needs at the heart of your support plan.

Before going home

We will discuss a leaving date with you and your circle of support at the beginning of your stay, and consider what needs to be done before you go home, for example, home visits, transport, getting the home ready (food, equipment etc). We will discuss any support needs you may have before leaving.

Private Facilities

Home First Residential is fully equipped to support and enable our guests to get back to independent living.

We have 30 bedrooms, furnished to a high standard to ensure your comfort and safety. Each has an en-suite bathroom, equipped to enable you to be as independent as possible. Hoists are available to assist you with bathing and there is a whirlpool type bath if you prefer.

Toilets are fitted with grab rails and raised toilet seats are available to enable you to use the facilities independently.

Each room has a lockable cupboard for your personal items and medication. You will be provided with a key to your own room.

Communal facilities

We have a communal lounge with TV, radio, CD player, books and board games. There is private garden to sit and relax in.

There is a cordless phone for you to receive calls. There is a charge for making outgoing calls and you are welcome to bring your own mobile phone or other communication devices.

A mobility scooter is available for you to use. You could use it to go to the local shops during your stay with us, or simply experience the benefits a scooter might bring to you when you get home.

We provide laundry facilities on site and you will be encouraged to do your own laundry or your family and friends can support you to do this.

You are welcome to access our guest wi-fi; please ask a member of staff to assist you. We also have a computer and iPads to enable you to keep in touch with family and friends and make use of the internet etc.

We work to a strict code of conduct, which includes preserving the confidentiality of all information that you tell us and all information we hold on you. We will not disclose it to anyone else without your permission, except in an emergency, which may include The Care Quality Commission.

If this happens we will keep you informed of any discussions that have taken place and this will be recorded in your care records, to which you may have access at any time.

This is in accordance with the Data Protection Act 2018 and General Data Protection Regulations.

The team includes social care staff, occupational therapists, physiotherapists, district nurses and general practitioners from social care and health. By working in an integrated way we are able to:

  • deliver support plans that bring together services to achieve the outcomes important to each individual
  • improve transition between health and social care services
  • communicate effectively to people accessing support services
  • ensure effective, timely and inclusive decision making between social care and health

We define ‘quality’ as delivering a care service appropriate to your individual needs. We have a self-assessment system that requires all of our procedures and work practices to be audited at least annually to ensure we maintain standards. Any non-conforming areas are actioned and reviewed to ensure that the issue is not repeated in the future. We publish the results on the communal notice-board.

The suitability and safety of the premises and facilities, including adaptations such as hearing loop systems, grab/hand rails, ramps, signs, and lighting, are assessed by appropriately qualified persons. Routine maintenance and renewal of furnishings and decoration of the premises is completed accordingly.

We are constantly learning from your experiences of our services, and value your thoughts and opinions to enable us to continually improve.

We welcome your comments at any time during your stay, and when you return home we ask you to complete a questionnaire about your experience of staying at The Intermediate Care Centre.

We will also ask your family and friends to complete a questionnaire to help us learn from their experiences.

Complaints

We don’t get things right all the time and we need you to tell us if we fall short of expected standards. If you have a complaint please speak to a senior staff member on duty. If the issue cannot be resolved to your satisfaction please ask to speak to the Manager and/or complete the North Lincolnshire Council complaints form [PDF, 86Kb].

If your complaint is still not satisfactorily resolved, you have the right to contact the Care Quality Commission.

If we do get anything wrong, we will inform you immediately, discuss this with you and keep you updated at every stage of the process of looking at what happened.
