North Lincolnshire council is seeking people’s views on how it delivers adult social care - in a bid to improve the service.
(The survey has now closed).
Service users, residents, parish councils, faith groups, leisure groups and voluntary organisations are among those being consulted.
The council wants to gather the views of a wide range of people in North Lincolnshire.
You do not have to be receiving support to be able to complete the survey.

The closing date for completed surveys is 12 April 2012.
Cllr Rob Waltham, cabinet member for adult and children’s services at North Lincolnshire Council, said:
“The council is reviewing the way it delivers the service and genuinely wants your input into helping shape things to come. We need your help to design services that better meet your needs and that will give you more control and freedom over the support you receive. Afterall, you are the ones best placed to tell us this.
“Listening to what you say is vital to ensure that in future, we deliver more targeted support that will give you greater independence. We believe people are entitled to live in their own homes for longer and we will do everything we can to help make this happen.
“People are living longer now and we must ensure that what we have in place is a service that people want and more importantly need.”
