You can now assess your care needs online to find out if we could help you. Adult Social Care Services are committed to helping people live safe and fulfilling lives in their own homes. We do this by providing support and equipment to those who are eligible. If you think you, or someone you know might benefit from our support you can use our online self assessment to find out if we can help you.
Giving you the choice
Self assessment gives people the choice to assess their own care needs or carry out an assessment on behalf of someone else online. This service is aimed at adults who are finding looking after themselves increasingly difficult. For example you or someone you know may be having difficulty getting in and out of the bath, managing stairs or with some aspects of personal care.
Providing a faster response
After you complete a self assessment your information will be sent to the Access, Response and Transfer Team who will contact you to organise an assessment based on the information you provided to determine whether you are eligible for services.
If it seems that your needs are more complex and we think you would benefit from a full assessment we will tell you this, or alternatively if you would like us to organise a full assessment for you instead of completing the self assessment, then you can request a full assessment online by clicking on the full assessment link. If we agree that you qualify for support, you will be able to choose from a range of services matching your assessed needs. If you would prefer we can arrange for you to have the cash equivalent for some of these services, so that you can arrange them yourself. This is called a Direct Payment.

Self assessment is open to people who live within the North Lincolnshire Council's boundary and are 65 years or over.

I understand that in order for North Lincolnshire Council, Adult Social Care Services to arrange the right services, the information I provide in the self-assessment may be shared with other professionals who may be involved in helping meet my needs for example District Nurse, Physiotherapist etc. By continuing with the self assessment, I give my consent to my information being shared for these purposes.
