The Intermediate Care Service (ICS) is a team of social and health care staff who work together to support people. It is known as an 'integrated multi-disciplinary team'. The ICS can help people:
- Stay at home while recovering from injury or illness
- Leave hospital more quickly following treatment
- In some cases, prevent the need to be admitted into hospital at all.
It does this by offering a programme of time-limited, short-term, rehabilitative support. Depending on the nature of the support required the help might be provided either:
- In your own home (home care support); or
- You may stay for a short while in one of the rehabilitation units: The Dove Suite, Lilacs, Scunthorpe or The Poirier Suite, De Lacy House, Winterton; or
- In a group dwelling ground floor flat at No. 7 Lincoln Court, Scunthorpe.
There is no charge for using the ICS. The team consists of:
- Team manager
- Senior care managers
- Intermediate care nurses
- Occupational therapists
- Physiotherapists
- Care workers
- Care managers
- General practitioner (GPs)
- Rehabilitation/technical instructors
This service is available if:
- You live in North Lincolnshire or your doctor's surgery is within the North Lincolnshire Primary Care Trust area.
- You are over 18 years old
- You are assessed as needing the Intermediate Care Service
- You are willing and able to take part in the rehabilitation programme
- Your need for the service is anticipated to be short term or time limited.
- You are medically stable
- You will be able to improve and achieve optimum independence
- You are able to agree to this
Asking for help - making a referral
You can request support from the ICS by making a referral. Telephone 01724 297979.
A referral simply means making the ICS aware of the circumstances of the person who needs help. Anybody can make a referral - you, your family, neighbours, a GP, district nurse, hospital staff, other professionals or anyone else.
Following the referral a member of the team will visit you as quickly as possible, usually within three hours, to talk to you about your situation and discuss your needs.
If it is agreed that the ICS can help you, a care and treatment plan is developed with you.
If you receive services at home, you will be given a folder called the ‘Service User Plan’, which you look after. It will include your care plan and other information showing you, team members, and your family the progress you are making. All staff visiting you will use your plan to chart your progress, and record their visits.
Please keep this folder safe. It's yours for as long as you receive our help.
Any information we hold about you is confidential and only shared with others who need to know, like your GP, and only after you have agreed to this. As your condition improves and your needs change, we will amend your plan accordingly.
What if I still need help after the Intermediate Care Service finishes?
If, after the service finishes you find you can’t manage, or your needs change again, tell us. This is so we can talk with you about how we can direct you to more help. We will leave you a contact number should you need us again in the future.
And if I have to go into hospital, what do I do about my pets?
Looking after your pets is your responsibility. You will need to make the arrangements for their care while you are in hospital. You may have a neighbour, family member or other friend who can do this or you may prefer to arrange for kennels or a cattery for the time you are away.
We may be able to help you make these arrangements if you need us to, on the understanding that the council cannot accept responsibility for any costs incurred, except in exceptional circumstances.
