People with dementia and their carers are eligible for a range of community based services to support them. An assessment will take place to understand what help and support is needed. People with more severe dementia and their carers may be directed to the specialist Integrated Community Mental Health Team for Older People. Memory assessment and treatment service
The Rotherham, Doncaster and South Humber Mental Health NHS Foundation Trust has a memory clinic based at Scunthorpe General Hospital. The clinic can offer general advice, education and support in connection memory problems. Please contact your GP if you would like to be referred to this service.
Dementia café
Adult Social Services, in partnership with the Alzheimer's Society, run a café in North Lincolnshire. The dementia café is a friendly place for people with dementia and their carers to meet other people in social situations, provide support to each other, share information and seek advice from the professionals that are on hand. The café is held every first Friday in the month, from 10am to 12pm and is based at:
