We have collected together a number of documents that are required by provider organisations on the Supporting People (SP) programme. These documents form part of the performance and quality returns to the Supporting People Team. There are also some documents that provide useful information.

This is the application form that providers use to apply for accreditation. All non-statutory organisations must be successfully accredited to deliver SP grant funded services within North Lincolnshire.

This is the QAF self assessment document for the six core service standards. Providers are required to complete this each year and submit to the SP team.

This is the QAF self assessment document for the eleven supplementary standards.

This document gives guidance about completing the QAF lite.

This is the QAF LITE self assessment document for the six core standards. This should be used by Community Alarms, sole traders and small providers if they employ less than one full time equivalent (FTE) member of staff, have a contract value of less than £5,000 per year and the weekly unit cost is less than £5.

This sheet should be used by all providers submitting QAF returns. It is a summary sheet of the standard assessed by the provider.

This work book should be used by providers to record statistical performance returns. It has to be submitted to the SP team quarterly, no later than two weeks after the end of the quarter. The reporting quarters are:
- 4/4/05 - 3/07/05
- 4/07/05 - 2/10/05
- 3/10/05 - 1/1/06
- 2/6/06 - 2/04/06.

This is the draft complaints and appeals policy. It details the process followed if a service user makes a complaint about a support provider or if a provider wants to complain about the practice/decisions of the SP programme.
