They support all early years practitioners with:

Early identification and subsequent support of children with additional needs

Planning for children with additional needs

Transition of children with additional needs into the pre-school and on into the maintained sector
The team offers practical support to practitioners as they work with and plan for children with additional needs within their settings, by:
§ offering ideas and strategies for planning
§ modelling strategies and resources within the settings
They work closely with North Lincolnshire’s Early Years SEN team, through:
§ Sharing relevant information
§ Joint Individual Education Plan (IEP) planning with the settings
§ Supporting the transition of children into the setting
The team supports all early years settings to establish effective working links with a wide range of agencies across health, education and social services.
