The application forms have been provided for you to download, print, complete and bring with you to your appointment at the Register Office. (Guidance notes are included with each application form).
The application forms are in Adobe Acrobat (PDF) format. You will need appropriate software to view and print. An Adobe Acrobat reader can be downloaded free from the Adobe website (full instructions for downloading the reader are provided on the website).
